This is a repeat violation, Class II. 113 CCR 2638(c), 2638(d) Failure to submit"Monitoring System Certification Form"within 30 days of completion of the test. Annual monitoring system certification, and leak detector testing, was performed on 6/27/18 and a test report was submitted on 8/6/18, over a month late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a repeat violation, Class II. 115 CCR 2715(a)(1)(B), 2715(a)(3) "Designated Underground Storage Tank Operator Identification Form" not submitted within 30 days. A new designated operator was hired on February 2019 and notification was not provided to the EHD within 30 days of the change. Any changes shall be reported to the EHD within 30 days. The manager logged into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and uploaded the notification identifying all the designated operators for this facility while the inspector was on site. This is a minor violation. 123 CCR 2637.1(d), 2637.1(e) Failure to submit the"Spill Container Testing Report Form"within 30 days after the test. Spill container testing was performed on 6/27/18 and a test report was submitted on 8/6/18, over a month late. A copy of the test report must be submitted within 30 days of the tests. This is a repeat violation, Class II. 201 CCR 2712(b)(1-2) Failure to maintain monitoring, maintenance, and follow up action records. Maintenance and monitoring records for the last three years were not found on site. secondary containment testing conducted on 6/26/18 were not found on site. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a repeat violation, Class II.
