The designated operator failed to document all the alarms from the attached alarm histories on the designated operator inspection reports and failed to check that they were responded to appropriately. The missing alarms include: L1 (diesel STP sump)fuel alarms on 12/5/18 and 10/3/18 L2 (diesel annular) high liquid alarms on 4/10/19, 12/20/18, and 5/31/18 During the visual inspection, the designated operator shall review the alarm history since the previous inspection, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the report. Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. A designated operator inspection was performed on January 20, 2019 and the next inspection was performed on February 21, 2019, 32 days later. The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. Ensure that all designated operator inspections are performed within the required time frames. The designated operator failed to check and note all required items on the visual inspection reports. The incorrect and missing information included the following from the inspection reports: -Overfill prevention test date was left blank. -The NA answers are not included in Section III. -The owner/operator signature was not included. -Tank and line integrity testing were left blank. -The question regarding UDC sensor locations was left blank. -Section V was left blank. During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a minor violation. 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. Numerous high liquid alarms occurred in the diesel tank annular throughout the last year. According to the work order on site, the high liquid alarms are due to water entering the tank annular space, raising the liquid levels. All monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. Immediately take all necessary steps to prevent water intrusion into the tank annular space. Verify that the water intrusion has not negatively impacted the functionality of the brine currently in the tank annular. This is a Class II violation.
