The missing alarms <br /> include: <br /> L1 (diesel STP sump)fuel alarms on 12/5/18 and 10/3/18 <br /> L2 (diesel annular) high liquid alarms on 4/10/19, 12/20/18, and 5/31/18 <br /> During the visual inspection, the designated operator shall review the alarm history since the previous inspection, <br /> check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the report. Ensure that designated operators performing <br /> inspections at this facility are including all of the required information on the reports. <br /> A designated operator inspection was performed on January 20, 2019 and the next inspection was performed on <br /> February 21, 2019, 32 days later. The designated operator visual inspections shall be conducted within 30 days or <br /> less of the previous inspection. Ensure that all designated operator inspections are performed within the required time <br /> frames. <br /> The designated operator failed to check and note all required items on the visual inspection reports. The incorrect and <br /> missing information included the following from the inspection reports: <br /> -Overfill prevention test date was left blank. <br /> -The NA answers are not included in Section III. <br /> -The owner/operator signature was not included. <br /> -Tank and line integrity testing were left blank. <br /> -The question regarding UDC sensor locations was left blank. <br /> -Section V was left blank. <br /> During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST <br /> system have been completed and shall check all required items on the report. Ensure that designated operators <br /> performing visual inspections at this facility are including all of the required information on the reports. <br /> This is a minor violation. <br /> 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> Numerous high liquid alarms occurred in the diesel tank annular throughout the last year. According to the work order <br /> on site, the high liquid alarms are due to water entering the tank annular space, raising the liquid levels. All <br /> monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. Immediately <br /> take all necessary steps to prevent water intrusion into the tank annular space.
