Selland Drive r <br /> Fresno , CA 93722 <br /> Re : UST Removal. 729 K Central Ave. , Tracy, CA. 95376 SR0078722 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of the <br /> application for a permit to remove the following underground storage tank (UST) system , including <br /> piping and associated appurtenances , at the above noted address : <br /> Tank # TA0520845: 100 gallon unknown tank <br /> The permit has been approved with the following conditions : <br /> 1 . Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements . <br /> 2 . Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Betty Ho at (209) 468-3455 to arrange the following inspection appointments (a 48- <br /> hour notice is required for inspection appointments) . <br /> a. Removal of tank and any applicable piping <br /> b. Witness soil and/or water sampling <br /> c. Verification of Tank ID number marked on the UST <br /> 4. If the removed backfill material and soil is found to be contaminated it must be disposed of at <br /> an approved disposal location . Treatment of soil onsite can only be performed under a <br /> hazardous waste treatment permit. A work plan and fees must be submitted to the EHD Site <br /> Mitigation Unit and the work plan must be approved before initiating any assessment or <br /> remediation activities. <br /> 5 . A copy of the submitted "Health and Safety Plan" must be on the jobsite for review. <br /> 6. See 'Sampling Protocol for Routine Tank Removals' and 'Verification Analysis' on page 6 and 7 <br /> of the permit application for required analytical tests and number of samples to be taken and <br /> witnessed by EHD. <br /> 7. A copy of the Tracy Fire Department permit must be submitted at the time of removal . <br /> 8. Inert UST with 30 pounds of dry ice per 1 , 000 gallon volume . Be aware, additional <br /> testing/monitoring to ensure tanks remain inerted may be required by the Tracy Fire <br /> Department. <br /> 1868 E.
