SAN ,dDAQ U I N Environmental Health Department <br /> C01-1 N T <br /> Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> D&D LIFT LLC 3820 RHONDA WAY, TRACY November 01, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 114 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br /> OBSERVATION: Designated facility signed copies of Uniform Hazardous Waste Manifest(UHWM)were not found <br /> on-site. <br /> Note: Operator could not provide any uniform hazardous waste manifest the past three years, 11-1-2016 to <br /> 11-1-2019. <br /> REGULATION GUIDANCE: A hazardous waste generator shall keep a copy of each manifest signed in accordance <br /> with section 66262.23(a)for three years or until the generator receives a signed copy from the designated facility <br /> which received the waste. This signed copy shall be retained as a record for at least three years from the date the <br /> waste was accepted by the initial transporter. The hazardous waste generator must submit an exception report to <br /> the California Department of Toxic Substances Control (DTSC)within 45 days if the destination facility signed <br /> manifest copy is not received from the transporter or designated facility. <br /> CORRECTIVE ACTION: Locate a copy of the destination facility signed manifests for used oil, antifreeze, oily <br /> solids, used oil filters and send a copy to the San Joaquin County Environmental Health Department(EHD). If the <br /> destination facility signed manifests cannot be found then prepare an exception report for each missing manifest as <br /> required by Title 22 California Code of Regulations section 66262.42. DTSC address is DTSC Report Repository, <br /> Generator Information Services Section, PO Box 806, Sacramento, CA 95812-0806. Send a copy of the exception <br /> report to DTSC and the EHD. <br /> This is a Class II violation. <br /> 118 HSC 25160.2(b)(3) Failed to keep copies of consolidated manifesting receipts for three years. <br /> OBSERVATION: Copies of consolidated manifest receipts from 11-1-16 to 11-1-19 for hazardous wastes including <br /> used oil, used antifreeze, used paper oil filters were not found on site during the inspection. <br /> REGULATION GUIDANCE: The generator shall retain each consolidated manifest receipt for at least three years. <br /> This period of retention is extended automatically during the course of any unresolved enforcement action regarding <br /> the regulated activity or as requested by the department or a certified unified program agency. <br /> CORRECTIVE ACTION: Immediately locate a copy of all consolidated manifest receipts from 11-1-16 to 11-1-19 <br /> and maintain them on site.
