The monitoring plan is not current and/or not approved by the EHD. -UDC leak sensor model was left blank. The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a repeat violation, Class II. 115 CCR 2715(a)(1)(B), 2715(a)(3)"Designated Underground Storage Tank Operator Identification Form" not submitted within 30 days. Dave Winkler is performing designated operator inspections at this facility and is not listed on the statement of designated operator. Any changes shall be reported to the EHD within 30 days. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. This is a repeat violation, Class II. 201 CCR 2712(b) Failure to maintain monitoring, maintenance, and follow up action records. Maintenance and monitoring records for the last three years were not found on site. -Overfill prevention test results for 1/25/19 were not found on site. These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. A copy of the results was emailed by the tester at the time of inspection. This was corrected on site. This is a repeat violation, Class II. 204 CCR 2712(1) Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a repeat violation, Class II.
