The designated operator failed to check and note all required items on the visual inspection reports. The incorrect and missing information included the following from the inspection reports: -Section V was left blank. -The owner/operator printed name was left blank. -Tank and line tightness testing was left blank. -The follow up action question was left blank. -The NA answers were not included in section III. During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. A designated operator inspection for the following months was performed more than 30 days: -Conducted 12/14/18, next conducted 1/17/19 (34 days) -Conducted 3/11/19, next conducted 4/11/19 (31 days) The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. Ensure that all designated operator inspections are performed within the required time frames. The designated operator failed to check and note the correct test dates for the secondary containment testing on the designated operator inspection reports. The previous test date of 8/16/17 is written on all reports. Secondary containment testing was conducted on 2/28/19. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. 320 CCR 2635, 2637.2, 2665, 2712 Failure to comply with the installation, inspection, or record keeping of the overfill prevention. Overfill prevention testing was conducted on January 25, 2019. This testing was required to be conducted by October 13, 2018. Ensure all future testing is conducted within required time frames. This is a Class II violation. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by June 21, 2019.
