The designated operator employee training was not <br /> performed or current . For facilities that are not routinely staffed, the designated operator shall implement an <br /> employee training program approved by the EHD. Ensure that employees have been trained by the designated <br /> operator, maintain the list on site, and submit a copy of the training records to the EHD. <br /> This is a minor violation. <br /> 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> The 91 north STP sump sensor was not located to detect a leak at the earliest opportunity. The sensor was located on <br /> the side opposite the fuel piping and was placed over a foot south of the lowest point. Monitoring equipment shall be <br /> maintained to be able to detect a leak at the earliest possible opportunity. The sensor was relocated to the lowest point <br /> of the sump by the service technician. Ensure that all monitoring equipment is maintained to detect a leak at the <br /> earliest opportunity. <br /> This is a repeat violation, Class II. <br /> 319 CCR 2635, 2637.1, 2665, 2712 No spill bucket, failed to contain 5 gallons, no means to keep it empty, or failed to be <br /> tested. <br /> The 91 north spill container failed to have a minimum capacity of five gallons. All spill containers shall have a <br /> minimum capacity of five gallons. Immediately contact a properly licensed, trained, and certified contractor to repair <br /> or replace the spill container under permit and inspection of the EHD. <br /> The 87 spill container failed when tested. All spill containers shall have a minimum capacity of five gallons and be <br /> capable of containing a spill or overfill until it is detected or cleaned up. Immediately discontinue deposition of <br /> petroleum into this tank until the component is repaired or replaced by a properly licensed, trained, aand certified <br /> contractor under permit and inspection by EHD. If the spill container cannot be replaced immediately, there is a <br /> possibility that the 87 UST system may be red tagged to prevent fuel inputs. <br /> This is a minor violation. <br /> 331 CCR 2630(d), 2641(a) Liquid does not flow into the sump in the event of a leak (i.e.,failure to remove test boot).. <br /> The plug under the clamshell on the 91 south STP sump product piping was not removed. The Schrader valve was <br /> open, but was pointed up. Any potential release from the primary containment into the secondary containment in the <br /> 91 south piping does not flow into a collection sump. All secondary containment system shall be sloped so that all <br /> release will flow to a collection sump located at the low point of the underground piping.
