The permit has been approved with the following conditions : <br /> 1 . Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements . <br /> 2 . Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work . All new addenda supersede the previous ones . <br /> 3 . Contact Paul Nso at (209) 468-3432 to arrange the following inspection appointments ( a 4 & <br /> hour notice is required for inspection appointments ) . Please be aware that inspections for <br /> emergency work must be scheduled within five days of permit approval . <br /> a . Overfill prevention verification : Measurement of drop tubes to verify 95 % tank volume shut <br /> off. Verification of ball floats absent or located at a level that will not interfere with drop tube <br /> functionality . <br /> Please be aware that the facility cannot receive fuel in the 91 tank prior to the EHD <br /> inspection and approval . <br /> 4 . Any individual performing the work of a service technician must meet the following <br /> qualifications : <br /> a . Possess or be employed by a person who possesses one of several permissible valid , <br /> current contractor' s licenses issued by the Contractors State License Board (may be issued <br /> to the installation company) , or a Tank Tester' s license issued to the individual by the State <br /> Water Board , and <br /> b . Possess or work under the direct and personal supervision of an individual physically <br /> present at the work site who possesses a valid , current California UST Service Technician <br /> certificate issued by the International Code Council ( ICC ) , and <br /> c . Possesses a valid , current certificate of training from the manufacturer(s ) of the <br /> component ( s) being tested , repaired , or serviced . <br /> 5 . Log into the California Environmental Reporting System ( CERS ) and verify the " UST Tank <br /> Information " form for 91 tank is current . <br /> 1868 E .
