Date:12/07/2020 <br />Manufacturer training: <br />OVERALL INSPECTION NOTES AND COMMENTS <br />NOTES <br />- Verified dispenser footprints contained by the UDCs. <br />- Verified installation of conversion frames and deflector plates. <br />- Visually inspected piping under dispensers - no leaks found. <br />While on site it was found that the monitoring panel had been cold started. All sensors showed a fuel alarm on April 18, 2019. <br />The alarm history attached to the April 11, 2019 designated operator inspection report showed an alarm history prior to the <br />cold start. Work orders did not indicate any cold start performed. The technician on site contacted the technician who <br />replaced the dispensers and confirmed that a cold start of the monitoring panel was performed on April 18, 2019 due to a <br />software upgrade. Immediately submit an amended scope of work for this permit to the EHD. <br />This facility is approved to operate. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
