Time In: 2:38 pm <br /> Time Out: 3:15 om <br /> Food Program Complaint Inspection Report <br /> Name of Facility: WALMART#1789 Date: 12/07/2020 <br /> Address: 1601 S LOWER SACRAMENTO RD, LODI 95242 <br /> Owner/Operator: WAL-MART Telephone: (209)368-6696 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00053083 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> COMPLAINANT ALLEGES THAT HE OBSERVED A PACKAGE OF HAMBURGER MEAT THAT CONTAINED BLOOD. <br /> COMPLAINANT ALLEGES HE ALSO OBSERVED BLOOD ON THE OTHER PACKAGES OF THE HAMBURGER MEAT. <br /> COMPLAINANT ALSO OBSERVED THE THREE PAPER TOWEL DISPENSERS NEARBY WERE EMPTY. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> I went to the facility and spoke to the manager who was not aware of the complaint prior to my investigation. <br /> Per manager, all meat comes to the facility pre-packaged and stored in the meat walk-in and then in the meat wall refrigerator <br /> on the sales floor. <br /> I observed the meat wall refrigerator on the sales floor. <br /> I observed packages of raw meat and raw hamburger meat. <br /> I did not observe"Blood on other packages of hamburger meat.." <br /> I did not observe blood on the exterior of meat packaging. <br /> I also observed sanitizer and paper towel stands that are set out approximately six feet apart in front of refrigeration <br /> cases. <br /> There were paper towels in one dispenser, another dispenser had paper towels but the paper towels were stuck, and one <br /> dispenser was out of paper towels. <br /> Paper towels were supplied to the empty dispenser and the dispenser that was not dispensing towels was adjusted so the <br /> towels dispensed properly. <br /> The dispensers were addressed during inspection. <br /> The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br /> Safety Code.If a reinspection is required,fees will be assessed at the current hourly rate. <br /> Received by: Name and Title: Sande, manager, sig not captured <br /> EH Specialist: STEPHANIE RAMIREZ Phone: <br /> FA0023528 C00053083 SCO04 12/07/2020 <br /> EHD 16-23 Rev.06/30/15 Page 1 of 1 Food Program Complaint Inspection Report <br /> 1868 E.
