The following is an itemized list of hazardous materials violations that have not been <br /> addressed for THE SERVICE STATION as of September 14, 2020. <br /> Open violations from August 01, 2018 inspection <br /> Violation#5-Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br /> site. <br /> 200 gallons of used oil, 200 gallons of used anti freeze, and 220 gallons of new oil were found on site that have not <br /> been reported. Any material that meets or exceeds the reportable quantity shall be reported in the facility's <br /> business plan. Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br /> approval. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#6-Failed to complete and/or submit a site map with all required content. <br /> The site map was not complete as part of the business plan. The site map is missing the storm drain locations, <br /> north orientation, emergency response equipment(fire extinguisher,first aid kit, spill kit), adjacent streets,and an <br /> evacuation area. A site map shall contain a north orientation, loading areas, internal roads, adjacent streets,storm <br /> and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material <br /> handling and storage areas, and emergency response equipment. If a site map element is not applicable for your <br /> facility then list it on the map and label as"NA". Immediately log into the California Environmental Reporting System <br /> (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#7-Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures were not complete. The local Unified Program Agency(UPA) phone number <br /> is missing. The local UPA phone number is 209-468-3420. Please fill out section F and J in the emergency <br /> response procedures.The locations for the brooms, shovels, phone, and absorbent were not identified on section G <br /> of the emergency response and contingency plan. List the locations. A business plan shall include the following <br /> emergency response procedures for a release or threatened release of hazardous materials, including, but not <br /> limited to,the following: <br /> -immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice,for the site <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> Page 1 of 2 <br />
