An approved copy of financial responsibility documents were not available for inspection. Financial documents on site are outdated. A copy of these documents shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a minor violation. 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. The following was observed on the designated operator's monthly inspection report: -The 10/25/18 designated operator inspection report was not signed by owner/operator within 48 hours. During the visual inspection, the designated operator shall check all required testing and maintenance for the UST system have been completed and shall check all required items on the report. Ensure that designed operators performing visual inspections at this facility are including all of the required information on the reports. This is a minor violation. 316 HSC 25290.1(c3), 25290.2(c3) Secondary containment not constructed to prevent water intrusion (after Jul 2003). A small amount of liquid was found in the 87-prouduct fill sump and 91-product STP sump. Secondary containment shall be constructed to prevent any water intrusion into the system by precipitation, infiltration, or surface runoff. The service technician removed approximately 2 oz of liquid from the both sumps and returned the sensors to their proper location. Immediately contact a properly licensed, trained, and certified contractor to address the water intrusion into the 87-prouduct fill sump and 91-product STP sump. This was corrected on site. This is a minor violation. Overall Inspection Comments: The monitoring system certification was conducted on 2/25/2019 and was not witnessed by EHD. Onsite to look inside all sumps and UDCs and verify sensor locations. Paperwork inspection was completed today, 6/12/2019. A complete report was provided at that time. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, within 30 days of receiving the complete inspection report.
