CALIFORNIA 95685 <br /> 209 267-9111 <br /> CONTRACTOR'S LICENSE No. 236050 <br /> ,lune 21 , 1990 <br /> ADDENDUM TO <br /> WOLIN & SONS , INC . <br /> SAFETY PROGRAM <br /> l . ) All excavations over five feet in depth shall be shored <br /> or sloped according to State of California, Cal/OSHA require- <br /> ments . <br /> 2 . ) All excavations to which the permit applies shall be under <br /> constant, competent supervision . <br /> 3 . ) Hard hats shall be worn in the excavation at all times . <br /> 4 . ) A ladder or other means of egress shall be located no <br /> more than 25 feet from all employees in the excavation. <br /> 5 . ) Spoil shall be removed at least two feet from the edge <br /> of the excavation. <br /> 6 . ) A].1 alternate shoring or sloping plan for an excavation <br /> deeper than 20 feet shall be designed and approved by a <br /> California Registered Engineer. <br /> 7 . ) If work involves a live sewer line or manhole or any other <br /> source of flammable or toxic gases or any possibility of <br /> oxygen deficiency, tests shall be made and recorded for these <br /> hazards before entering the excavation. If a hazard is <br /> discovered, suitable means shall be used to mitigate the <br /> hazard as required by the California Safety Orders . <br />
