The missing alarms <br /> include: <br /> -1_2 (diesel STP sump) sensor out alarms on 11/20/18 <br /> -1_5 (diesel annular) low liquid alarm on 11/27/18 and high liquid alarm on 9/18/18 <br /> -Q1 (91 PLLD)gross line fail on 2/15/18 <br /> -Q3 (87 PLLD)gross line fail on 10/24/18, 7/6/18, and 6/5/18 <br /> -The designated operator did not include any notes on how the 7/26/18 and 7/25/18 L5 (diesel annular) high liquid <br /> alarms were responded to; the alarm logs on site state the designated operator"reset"the sensor during the <br /> inspection. <br /> -The April 2018 report states the L1 (91 STP sump) liquid warning alarm on 4/6/18 cleared on its own; the alarm logs <br /> on site and the May 2018 report state rain water was removed by the site. <br /> -The April 2018 report incorrectly states that"sensors in tank top sumps have not alarmed" since the last inspection. <br /> The 91 STP sump sensor alarmed on 4/6/18. <br /> During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> The designated operator failed to check and note all required items on the visual inspection reports. <br /> -Two copies of the August monthly report were found on site. One was undated, the other had a date of 8/10/18. The <br /> alarm history printout on site was dated 8/21/18. <br /> -Overfill prevention test information was left blank on the 12/18/18 inspection report and was not documented in <br /> Section III. <br /> -Overfill prevention testing was marked as a 'no' on the 10/31/18 inspection report and was not documented in Section <br /> III. <br /> -Section V was left blank on the 10/31/18, 11/27/18, and 12/18/18 reports. None were signed by the owner/operator. <br /> This section is required to be completed by the site owner or operator within 48 hours of receiving the inspection <br /> report. <br /> During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST <br /> system have been completed and shall check all required items on the report. Ensure that designated operators <br /> performing visual inspections at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 320 CCR 2635, 2637.2, 2665, 2712 Failure to comply with the installation, inspection, or record keeping of the overfill <br /> prevention. <br /> Overfill prevention testing was conducted on 1/15/19. This testing was required to be conducted by October 13, 2018. <br /> Ensure all future testing is conducted within required time frames. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> FA0003994 PR0231785 SCO01 01/15/2019 <br /> EHD 23-01 Rev.09/22/16 Page 6 of 7 Underground Storage Tank Program OIR <br />
