Overfill prevention equipment <br /> inspection report conducted on 8/24/18 was not on site. These records shall be maintained on site for at least three <br /> years. Monitoring records include: (1)date and time of all monitoring or sampling; (2)monitoring equipment <br /> calibration and maintenance records; (3)results of any visual observations; (4)results of sample analysis performed <br /> a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment,ground water elevations, or <br /> other test results; (6)results of inventory readings and reconciliations. Immediately locate and maintain all <br /> maintenance and monitoring records for the last three years on site and submit copies to the EHD. <br /> This is a Class II violation. <br /> 301 HSC 25292.1(a) UST system is not operated to prevent unauthorized release, including spills and overfills. <br /> Fuel filters were observed outside of all under dispenser containments at the facility.The underground storage tank <br /> system shall be operated to prevent unauthorized releases. Immediately contact a properly licensed,trained, and <br /> certified contractor to replace or upgrade the component under permit and inspection with the EHD. <br /> This is a Class II violation. <br /> 314 HSC 25290.1, 25290.2, 25291 Failed to have secondary containment or secondary containment not tight. <br /> Fuel filters were observed outside of all under dispenser containments at the facility. The underground storage tank <br /> system shall be operated to prevent unauthorized releases. Immediately contact a properly licensed,trained, and <br /> certified contractor to replace or upgrade the component under permit and inspection with the EHD. <br /> This is a Class II violation. <br /> 319 CCR 2635, 2637.1, 2665, 2712 No spill bucket,failed to contain 5 gallons, no means to keep it empty, or failed to be <br /> tested. <br /> The 91 product spill container was tested for 60 minutes using the hydrostatic method with passing results however <br /> failed to have a minimum capacity of five gallons. All spill containers shall have a minimum capacity of five gallons. <br /> Immediately contact a properly licensed,trained, and certified contractor to repair or replace the spill container <br /> under permit and inspection of the EHD. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by April 25th, 2019. <br /> Document reviewed: Monitoring system certification test results from 2016, 2017, 2018 Secondary containment <br /> 2019, employee training, response plan, maintenance records, monitoring plan, financial responsibility <br /> documents and designated operator monthly inspection reports from April 2018- March 2019. <br /> Documents provided: Return to compliance certification. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
