SANJ OA U I Environmental Health Department <br /> COUNTY <br /> — <br /> underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> VANCO TRUCK-AUTO PLAZA 1 1033 W CHARTER WAY, STOCKTON June 26, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. <br /> A designated operator inspection was performed on November 29, 2018 and the next inspection was performed on <br /> December 31, 2018, 33 days later. The designated operator visual inspections shall be conducted within 30 days or <br /> less of the previous inspection. Ensure that all designated operator inspections are performed within the required <br /> time frames. <br /> The designated operator failed to check and note all required items on the visual inspection reports. <br /> -Monitoring system certification date was incorrectly stated, correct test date is 6/20/2018. <br /> -Overfill prevention testing date was incorrectly stated, correct test date is 2/28/2019. <br /> -The 10/29/2018 inspection report was completed on the old form. <br /> -Tank and line tightness testing information was left blank. <br /> -Reports from October to January were signed by the designated operator instead of the UST owner/operator. <br /> -Reports were missing name and signature of the UST owner/operator. <br /> -The question regarding UDC and sump sensor locations was left blank. <br /> During the visual inspection, the designated operator shall check all required testing and maintenance for the UST <br /> system have been completed and shall check all required items on the report. Ensure that designed operators <br /> performing visual inspections at this facility are including all of the required information on the reports. <br /> This is a minor violation. <br /> 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of (pre-Jul 2003). <br /> Liquid was observed in the UDC Dispenser 7/8, 15/16 and 14 . If water could enter into the secondary containment <br /> by precipitation or infiltration, it must be removed and disposed of properly. Ensure that all sumps and annular <br /> spaces are maintained free of liquid. <br /> This is a minor violation. <br /> 319 CCR 2635, 2637.1, 2665, 2712 No spill bucket, failed to contain 5 gallons, no means to keep it empty, or failed to <br /> be tested. <br /> The Diesel #2 spill container failed when tested. All spill containers shall have a minimum capacity of five gallons <br /> and be capable of containing a spill or overfill until it is detected or cleaned up. Immediately discontinue deposition of <br /> petroleum into this tank until the component is repaired or replaced by a properly licensed, trained, and certified <br /> contractor under permit and inspection of the EHD. If the spill container can't be replaced immediately, there is a <br /> possibility that the [GRADE] UST system may be red tagged to prevent fuel inputs. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by July 26, 2019. <br /> FA0003829 PR0232352 SCO01 06/26/2019 <br /> EHD 23-01 Rev.09/28/2018 Page 5 of 6 Underground Storage Tank Program OIR <br /> 1868 E.
