The monitoring plan is not current and/or not approved by the EHD. The monitoring plan stated the Monitor Panel Model to be TLS-350 however a TLS-350PIus was observed while the inspector was on site. The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a repeat violation, Class II. 106 CCR 2632(d)(2), 2634(e) Failed to have an approved UST response plan. The response plan is not current and/or not approved by the EHD. The response plan that was found on site and uploaded to CESR state Mark Hammatt and Ismael Ramirez were responsible persons however Per Jose Jimenez, these people no longer are with the company. The response plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, upload the completed form, and submit for review by the EHD. Ensure a copy is available for inspection at any time. This is a minor violation. 113 CCR 2638(c), 2638(d) Failure to submit"Monitoring System Certification Form"within 30 days of completion of the test. A monitoring system certification, and leak detector testing were performed on 7/17/18 and communication was tested on 10/19/18. A test report has not been submitted to the EHD or the California Environmental Reporting System(CERS). A copy of the test report must be submitted within 30 days of the test. Immediately provide this report to the EHD. This is a minor violation. 121 CCR 2712(f) Failed to submit a written response within 30 days of receiving an inspection report. An inspection was last done on 7/17/19 and an inspection report was issued identifying information to be submitted to bring this site into compliance. This information was required to be submitted by 8/17/19. This information was received on 8/23/19 resulting in a non-compliant status for this facility. An operator that receives an inspection report shall have 30 days to submit a written response that includes a statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. This is a repeat violation, Class ll. 123 CCR 2637.1(d), 2637.1(e) Failure to submit the "Spill Container Testing Report Form"within 30 days after the test. Spill container testing were performed on 7/17/18 and communication was tested on 10/19/18. A test report has not been submitted to the EHD or the California Environmental Reporting System (CERS). A copy of the test report must be submitted within 30 days of the test. Immediately provide this report to the EHD. This is a minor violation.
