The City will create an incident file as part of the illicit discharge investigation <br /> and corrective action process which will include the following documentation: <br /> i. Initial discovery of the illicit discharge(i.e. who discovered the <br /> discharge, date &time of discovery,and other pertinent initial details). <br /> (Spill/Illicit Discharge—Initial Contact Form) <br /> ii. Analytical data(if applicable). <br /> iii. Documentation of 24 hour response for suspected SSOs or significantly <br /> contaminated non-storm water flows. (SpilUIllicit Discharge—Initial <br /> Investigation Form) <br /> iv. Referrals to OES, County Department of Environmental Health, or other <br /> agencies. (Director of Public Works or designee) <br /> v. Documentation of notice and order to abate or emergency orders and <br /> abatements. <br /> A. Documentation of any administrative citations. <br /> vii. Verification of elimination of illicit discharge/connection and corrective <br /> actions through a follow-up inspection within 24/72 hours. (Spill/Illicit <br /> Discharge—Follow-up Investigation Form) <br /> viii. Document actions taken for investigation that require more than 72 <br /> hours to identify and locate the source of a suspected illicit discharge. <br /> ix. Turn in all forms and documentation to the Storm Water Coordinator. <br /> x. Storm Water Coordinator should review documentation to verify <br /> complete documentation. <br /> A. Incorporate incident into the storm water database. (Storm Water <br /> Coordinator or designee) <br /> 4. Reference <br /> a. City of Lathrop Storm Water Management and Discharge Control Ordinances <br /> (§13.28) <br /> b. City of Lathrop Spill Response Plan <br /> c.
