A roll-off bin containing solid sludge with arsenic is not identified or labeled as hazardous waste. <br /> All hazardous waste containers shall be marked with the following information: <br /> 1. the words "Hazardous Waste", 2. name and address of generator, <br /> 3. hazardous properties (toxic, corrosive, reactive or ignitable) 4. physical state (solid or liquid) <br /> 5. composition (contents), 6. accumulation start date (date when arsenic sludge was placed inside) <br /> Immediately place a sign for the roll-off bin and ensure that all hazardous waste containers are marked <br /> with all the required information. <br /> Submit to SJC EHD within 30 days (06/13/14): <br /> 1. one copy of the completed "Return to Compliance Certification" form <br /> 2. a written corrective action statement for the above violations <br /> 3. requested/ supporting documentations ie. manifest records <br /> Notes- <br /> -Outside contractor maintains corp yard's vehicle fleet: no used oil, filters, antifreeze. <br /> -Outside contractor Unico Engineering Tony Ouellette is supervising operations of water treatment.
