The designated operator failed to check and note all required items on the visual inspection reports. The incorrect and missing information included the following from the inspection reports: -Section V was left blank. - Section VI was unsigned by owner/operator. -The reports indicate that containment sumps were being inspected by designated operator but does not list any containment sumps. During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. A designated operator inspection for the following months was performed more than 30 days: -Conducted 12/13/19, performed on 1/18/19 (36 days late) -Should have been conducted 1/12/19, performed on 2/15/19 (34 days late) -Should have been conducted 3/11/19, performed on 3/15/19 (32 days late) -Should have been conducted 4/10/19, performed on 4/15/19 (35 days late) The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. Ensure that all designated operator inspections are performed within the required time frames. This is a minor violation. 313 HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. Liquid was observed in UDC 7/8 and a fuel drip was observed on a the south-most fuel filter in dispenser 7/8. All primary containment for the UST system must be tight. Immediately have a properly licensed, trained, and certified contractor repair or replace the failed component under permit and inspection of the EHD. This is a repeat violation, Class II. Overall Inspection Comments: The monitoring system certification is being conducted on 4/9/19 and was not witnessed by EHD. Looked inside all sumps and UDCs. Verified sensor locations and piping communication. Verified spill bucket 5 gallon capacity. The paperwork inspection was conducted on 5/3/19. A complete report was left at that time. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by June 2nd, 2019.
