<br />The designated operator failed to check and note all required items on the visual inspection reports.
<br />-The 3/27/19, 2/21/19, 1/24/19, 12/26/18, 11/28/18, 10/24/18 report did not include CERS ID.
<br />-The 3/27/19, 2/21/19, 1/24/19, 12/26/18, 11/28/18 report did not include the N and NA answers in section III.

<br />-The 11/28/18 report was unsigned by the owner/operator.

<br />-The 10/24/18 report left blank the secondary containment testing question.
<br />-The 3/27/19, 2/21/19, 1/24/19, 12/26/18, 11/28/18 report incorrectly stated the secondary containment testing was
<br />not conducted on time, but included an accurate test date.
<br />-The 3/27/19, 2/21/19, 1/24/19, 12/26/18, 11/28/18, 10/24/18 report left blank the question regarding UDC sensor
<br />locations.
<br />-The 3/27/19, 2/21/19, 1/24/19, 12/26/18, 11/28/18, 10/24/18 report left blank the tank tightness testing and line
<br />tightness testing questions.
<br />During the visual inspection, the designated operator shall check that all required testing and maintenance for the
<br />UST system have been completed and shall check all required items on the report. Ensure that designated
<br />operators performing visual inspections at this facility are including all of the required information on the reports.
<br />-A designated operator inspection was performed on 10/24/18 and the next inspection was performed on 11/28/18,
<br />35 days later.
<br />-A designated operator inspection was due by 11/23/18 and the next inspection was performed on 12/26/18, 33
<br />days later.
<br />-A designated operator inspection was due by 12/23/18 and the next inspection was performed on 1/24/19, 32 days
<br />later.
<br />-A designated operator inspection was performed on 2/21/19 and the next inspection was performed on 3/27/19, 34
<br />days later.
<br />The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection.
<br />Ensure that all designated operator inspections are performed within the required time frames.
<br />This is a repeat violation, Class II.

<br />CCR 2716(f) DO monthly reports not maintained on site for 12 months or for 36 months (after October 1, 2018).
<br />The April 2018 designated operator monthly inspection report was not found on site. Designated operator monthly
<br />inspection reports for the previous twelve months shall be retained on site. Locate and ensure that copies of the
<br />previous twelve months of designated operator monthly inspection reports are maintained on site. Submit a copy of
<br />the missing report to the EHD.
<br />This is a minor violation.
