The permit has <br /> been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Since the functionality of the monitoring system was already verified during routine <br /> testing on October 23, 2018 no further testing is required. <br /> • Sensor functionality: Functionality of the monitoring system must be verified to ensure <br /> that monitoring has been restored to meet the requirements of Title 23 California <br /> Code of Regulation and the Health & Safety Code. If it is determined that a cold start <br /> is necessary, a copy of the alarm history shall be printed prior to performing the "cold <br /> start" of the monitoring system and made available at the time of EHD inspection <br /> Please be aware that the facility cannot operate the UST system prior to the EHD <br /> inspection and approval. <br /> 4. If concrete will be broken near the tank, a secondary containment test must be <br /> performed on the tank once concrete is poured in accordance with the manufacturer's <br /> guidelines. <br /> 5. Any individual performing the work of a service technician must meet the following <br /> qualifications: <br />
