The following i an itemized list of hazardous waste violations that have not been <br /> addressed for 24 HR Gas & Mart as of February 17, 2015. <br /> Open violations from August 13, 2014 inspection <br /> Violation #109 -Failed to keep signed copy of manifests from the designated facility for three years. <br /> Copies of uniform manifes s for the disposal of hazardous waste were not found on site. According to work orders a <br /> Mr. Saleem Hussain, the f cility has generated both liquid and solid hazardous waste. Manifest number <br /> 005479303FLE (1/23/13) as listed on the DTSC hazardous waste tracking system, but was not found on site. <br /> Hazardous waste generat rs shall retain copies of all manifests for three years or until a copy signed off by the <br /> destination facility is receiv d. The signed off copy shall be retained and made readily available for review for three <br /> years. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator <br /> the facility to which the ge erator's waste was submitted within 60 days of the date the waste was accepted by the <br /> initial transporter, the gene ator shall submit a legible copy of the missing manifest, with some indication that the <br /> generator has not received confirmation of delivery. This information shall be submitted to: <br /> DTSC Report Reposito <br /> Generator Information ervices Section <br /> P.O. Box 806 <br /> Sacramento, CA 9581 -0806 <br /> Immediately locate a copy f the missing manifests or prepare and submit the required information to DTSC. Submi <br /> to the EHD a copy of the si ned off copy of the manifests or a copy of the information submitted to DTSC. This is a <br /> Class II violation. <br /> Violation#110 -Failed to omply with uniform hazardous waste manifest exception requirements. <br /> Manifest number 0054793 3FLE(1/23/13)was not found on site. Hazardous waste generators shall retain copies of <br /> all manifests for three year or until a copy signed off by the destination facility is received. The signed off copy shall <br /> be retained and made read ly available for review for three years. If the generator did not receive a copy of the manil <br /> with the handwritten signat re of the owner or operator of the facility to which the generator's waste was submitted <br /> within 35 days of the date t e waste was accepted by the initial transporter, the generator shall contact the transports <br /> and/or the owner or operat r of the designated facility to determine the status of the waste. If the generator did not <br /> receive a copy of the signe off copy of the manifest within 45 days of the date that the waste was accepted by the <br /> initial transporter, the gener for shall submit an Exception Report that includes a legible copy of the missing manifes- <br /> and a cover letter signed b the generator explaining the efforts to locate the hazardous waste and the results of tho; <br /> efforts. This is <br /> Class II violation. <br /> Violation #715 -Failed to eep complete records of each shipment of universal waste for three years. <br /> Copies of universal waste di posal records for were not found on site. Universal waste disposal records shall be <br /> retained for three years and have them readily available for review. Immediately locate a copy of all universal waste <br /> (lamps, batteries, aerosol c ns, CRTs, CRT glass, electronic waste, mercury containing equipment) disposal receipt: <br /> for the last three years, mai tain them on site, and submit copies to the EHD.
