Manifest number 005479303FLE <br /> (1/23/13)was not found on site. Hazardous waste generators shall retain copies of all manifests for three years or until <br /> a copy signed off by the de tination facility is received. The signed off copy shall be retained and made readily available <br /> for review for three years. I the generator did not receive a copy of the manifest with the handwritten signature of the <br /> owner or operator of the fa ility to which the generator's waste was submitted within 35 days of the date the waste was <br /> accepted by the initial trans orter, the generator shall contact the transporter and/or the owner or operator of the <br /> designated facility to deter ine the status of the waste. If the generator did not receive a copy of the signed off copy of <br /> the manifest within 45 days of the date that the waste was accepted by the initial transporter, the generator shall submit <br /> an Exception Report that in ludes a legible copy of the missing manifest, and a cover letter signed by the generator <br /> explaining the efforts to loc to the hazardous waste and the results of those efforts. Box 806 <br /> Sacramento, CA 95812 0806 <br /> Immediately locate a copy f the missing manifests or prepare and submit an exception report to DTSC. Submit to the <br /> EHD a copy of the signed o copy of the manifests or a copy of the exception report submitted to DTSC. This is a Class <br /> II violation. <br /> 608 Failed to properly manage sed oil and fuel filters. Bills of lading for the recycling of used oil filters for the last three <br /> years were not found on sit . According to a work order dated 5/9/14, the technician changed a fuel filter and "left filter <br /> in bag next to trash can." N used filters were observed on site. A copy of each bill of lading must be kept on site for at <br /> least three years. Immediately locate a copy of all bills of lading for used oil and gasoline filters for the last three years, <br /> maintain them on site, and ubmit copies to the EHD. This is a Class II violation. <br /> 713 Failed to keep complete recrds of each shipment of universal waste for three years. Copies of universal waste <br /> disposal records for were r t found on site. Universal waste disposal records shall be retained for three years and have <br /> them readily available for re tiew. Immediately locate a copy of all universal waste (lamps, batteries, aerosol cans, <br /> CRTs, CRT glass, electrons waste, mercury containing equipment) disposal receipts for the last three years, maintain <br /> them on site, and submit co ies to the EHD. This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy o the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective ac ions that have been or will be taken for each violation, and any supporting <br /> paperwork, by September 12, 014 . <br /> Consent to perform the inspec ion, take photos and collect samples was given by Saad Alsanay, manager. <br /> This facility generated an unkn wn amount of waste in the last calendar year (Jan-Dec). <br /> Please be aware as of Janua 1, 2013, all businesses are required to submit all hazardous materials information <br /> online to the California Environ mental Reporting System (CERS) at http://cers.calepa.ca.gov.
