Please find my recommendations for revising <br /> this report detailed below. <br /> There are three required items which were not covered by this <br /> plan: training requirements, medical surveillance program, and <br /> documentation. I have attached a copy of the site safety plan <br /> guidance for your information and for assistance to the <br /> contractor in supplying the additional information. <br /> Specific comments on various portions of the plan are as follows: <br /> Page 3 , Section 5. 3 - How many people are anticipated to be <br /> working at the site? Depending on size of field crew, the PMSSO <br /> may need to be split into two jobs. <br /> Page 4 , Section 5. 4 - Background information needs to include a <br /> summary/table of analytical data to date and some indication of <br /> sampling locations relative to proposed activities. <br /> Page 4 , Section 5. 5 - Needs to include directions to the hospital <br /> and a map. The list of emergency contacts should include a DHS <br /> contact. <br /> Page 4 , Section 5. 5 . 2 - Procedures need to include directions on <br /> emergency decontamination. <br /> Page 6, Section 5 . 6. 2 - Need to add information on dormant spray <br /> oil. <br /> Page 6, Section 5 . 6. 4 - What type of respirator - half-mask or <br /> full-face? Need to add liners (surgical gloves) to list of <br /> equipment. <br /> Page 6, Section 5. 7 - Need to add items on personal hygiene and <br /> respirator use. <br />
