S A N J 0 A Q I I I Irl Environmental Health Department <br /> CCUNT <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> PAQ Inc DBA Rancho San Miguel Market 620 S Cherokee Ln, Lodi September 16, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 303 CCR 66262.34(d)(2) Failed to maintain all communication or alarm systems, spill control, or decontamination <br /> equipment. <br /> OBSERVATION: The emergency eyewash station located near the hazardous waste storage area did not have <br /> eyewash solution in the cartridge. <br /> REGULATION GUIDANCE:All facility communications or alarm systems,fire protection equipment, spill control <br /> equipment, and decontamination equipment,where required, shall be tested and maintained as necessary to assure <br /> its proper operation in time of emergency. <br /> CORRECTIVE ACTION: Bring the emergency eyewash station into proper working order by repair, replacement or <br /> testing. Provide a corrective action statement to the San Joaquin County Environmental Health Department within <br /> 30 days. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Notes: <br /> 1. OBSERVATION: A hazardous waste container was observed on-site with incomplete labeling. The container <br /> was a blue poly drum storing used absorbent contaminated with laundry detergent, and was labeled with the <br /> incorrect generator address. <br /> REGULATION GUIDANCE: All hazardous waste containers shall be marked with the following information: the <br /> words "Hazardous Waste", name and address of generator, hazardous properties, physical state, composition <br /> (contents), accumulation start date <br /> CORRECTIVE ACTION: This issue was corrected during the inspection by Joann Villalpando, Store Director, <br /> who corrected the generator address on the hazardous waste container label at the time of inspection. No <br /> further action is required to correct this issue. <br /> 2. The facility did not have any hazardous waste disposal records for 2019. According to Joann Villalpando, the <br /> facility did not generate or dispose of any hazardous waste in 2019. <br /> 3. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by October 16, 2020. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
