Eight Mile Rd. Stockton Ca 95219 Program: HW SUMMARY OF VIOLATIONS (CLASS I, CLASS II,or MINOR-Notice to Comply) On site to conduct routine inspection. Minor-Notice to Comply: 8, 23(5), 60, & 63. #8 At time of inspection this facility does not have an active epa id #. Generators of hazardous waste must have an epa id #. Correct immediately. Submit a copy of this facility's active epa id # to this office by 6-17-10. #23(5) At time of inspectio the bill of lading for used filters were not available for review. I observed work orders from Holt date July 24, 2009 and May 17, 2009 that indicated that fuel and/or oil filters were changed out. Bill of la ing for used filters must be retained for 3 years. Correct immediately. Submit copies of these rec rds to this office by 6-17-10. #60 & 63 At time of inspecti n the manifests for hazardous waste disposal were not available for review. I observed work orders fron) Holt dated July 24, 2009 and May 17, 2009 that indicated that oil was changed out. Manifests for Oazardous waste must be retained for 3 years. Correct immediately. submit copies of these records to this office by 6-17-10. Complete and submit the returnto compliance certification form, corrective action statements for each violation, and any supporting documentation to this office by 6-17-10. Note: A copy of the CUPA classes & DTSC fact sheet on universal waste management was provided. ALL EHD STAFF TIME ASSOCIATED WI H FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($115).
