The <br /> permit has been approved with the fallowing conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior <br /> to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Elena Manzo at (209) 953-7699 to arrange the following appointments (a 48-hour <br /> notice is required for inspection appointments). <br /> • Sensor functionality: Functionality of the monitoring system must be verified to ensure <br /> that monitoring has been restored at the 89 and 91 tank annular sensors to meet the <br /> requirements of Title 23 California Code of Regulations and the Health & Safety Code. <br /> If it is determined that a cold start is necessary, a copy of the alarm history shall be <br /> printed prior to performing the "cold start" of the monitoring system and made available at <br /> the time of EHD inspection. <br /> • Since concrete will be broken near the 89 and 91 tanks, a secondary containment test <br /> must be performed on these tanks once concrete is poured in accordance with the <br /> manufacturer's guidelines. <br />
