The permit <br /> has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior <br /> to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Elena Manzo at (209) 953-7699 to arrange the following appointments (a 48-hour <br /> notice is required for inspection appointments). <br /> • Verification of the dispenser footprints contained by the under dispenser containment <br /> sumps (UDCs). <br /> • Visual inspection of the piping within the dispensers for any leaks. <br /> • Verification of the installation of the Bravo CONV-132000 conversion frame per <br /> manufacturer instructions. <br /> • Sensor functionality: Functionality of the monitoring system must be verified to ensure <br /> that monitoring has been restored to meet the requirements of Title 23 California Code <br /> of Regulations and the Health & Safety Code. A copy of the alarm history shall be <br /> printed prior to performing the "cold start" of the monitoring system and made available <br /> at the time of EHD inspection. <br /> Please be aware that the facility cannot operate the UST system prior to the EHD <br /> inspection and approval. <br />
