Main Street <br /> Stockton, CA 95202-3029 ENVIRONMENTAL HEALTH <br /> PERMIT/SERVICES <br /> RE: Hazardous Waste Program Inspection Reports <br /> AutoZone Stores located in San Joaquin County, CA <br /> Dear Ms. Henry: <br /> Through this letter, AutoZone is providing the additional information as requested in our <br /> recent telephone conversation. <br /> 1. A typographical error on page 1 of AutoZone's letter dated March 25, 2001 was addressed <br /> via a fax to your office on April 1,2011. An additional copy of the revised page 1 of that <br /> letter and the fax transmission confirmation page is attached herein. <br /> 2. An original signed Return to Compliance form for AZ store#3315 was included in <br /> AutoZone's letter dated March 25,2011. A copy of that Return to Compliance form is <br /> attached herein. <br /> 3. AutoZone's letter dated March 25, 2011 made reference to a letter of interpretation <br /> (RCRA Monthly Call Center Report, June 2004),but did not include a copy of the <br /> document. Per your request,a copy is attached herein. The document can be found at: <br /> http://www.el2a.gov/osw/hazard/wasteiypes/universaVlamps/call-g-a.pdf. <br /> 4. The log for universal lamps provided in the AutoZone letter dated March 25, 2011 was <br /> determined to be inadequate. A revised log titled"Sylvania Lamp Recycling by <br /> AutoZone Store Location(5-01-09 through 3-31-11)"is attached herein. <br /> 5. The San Joaquin County Office of Emergency Services allows electronic submittal of a <br /> very basic contingency plan. A complete Emergency Response Contingency Plan is <br /> maintained in the hard copy of the HMBP at each store. The following revisions were <br /> made to the Emergency Response Contingency Plan for all stores. An example of the <br /> revised HMBP for AutoZone Store#2858 is attached herein. HMBPs at all other San <br /> Joaquin stores were revised accordingly. <br /> ■ An Emergency Equipment Inventory Table was added. <br /> ■ The Incident vs. Response Section was revised to clearly identify the Store Manager <br /> as the On-Scene Emergency Coordinator and to provide his/her name and multiple <br /> telephone contact numbers. <br />
