Immediately log into CERS, <br /> update the required information, and submit for review by the EHD. <br /> This was corrected on site. <br /> This is a Class II violation. <br /> 115 CCR 2715(a)(1)(B),2715(a)(3)"Designated Underground Storage Tank Operator Identification Form"not submitted <br /> within 30 days. <br /> Stephanie Murphy is performing designated operator inspections at the facility and is not listed on the submitted <br /> statement of designated operators. Notification was not provided to the EHD within 30 days of the change.Any <br /> changes shall be reported to the EHD within 30 days. Immediately log into the California Environmental Reporting <br /> System (CERS)at http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for <br /> this facility. <br /> This is a minor violation. <br /> 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. <br /> The following was observed on the designated operator's monthly inspection reports: <br /> -Section VI of the report was missing date, name and signature of the owner/operator. <br /> During the visual inspection,the designated operator shall check all required testing and maintenance for the UST <br /> system have been completed and shall check all required items on the report. Ensure that designed operators <br /> performing visual inspections at this facility are including all of the required information on the reports. <br /> This is a minor violation. <br /> 318 CCR 2666(c), 2643(c)(1)Leak detector failed to detect a 3.0 gallons per hour leak at 10 pounds per square inch. <br /> The 87 and 91 product VMI line leak detectors failed to detect a leak when tested. All line leak detectors shall be <br /> capable of detecting a 3-gallon per hour leak at 10 psi.
