Manifest number 013425 16JJK (1/21/16)was found without a signed off copy from the destination facility. Hazardous waste generators shall re ain copies of all manifests signed off by the destination facility on site for three years and have them readily availab a for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or perator of the facility to which the generator's waste was submitted within 60 days of the date the waste was acce ted by the initial transporter, the generator shall submit a legible copy of the missing manifest, with some Indic tion that the generator has not received confirmation of delivery. This information shall be submitted to: DTSC Report Reposit ry Generator Information ervices Section P.O. Box 806 Sacramento, CA 9581 -0806 Immediately locate a cop of the missing manifest or prepare and submit the required information to DTSC. Submit to the EHD a copy of the sigi led off copy of the manifest or a copy of the information submitted to DTSC. One box of DPF ash was observed with an accumulation start date of /18; according to Mr. Nordberg, this is from June 18, 2015. Facilities who generate less than 1000 kg of hazardous Iwaste per month and do not exceed 6000 kg of waste stored on site at any time may store waste on site up to 180 da s.
