Financial responsibility documents have not been submitted to the EHD. The most recent financial responsibility documents in CERS are dated 2/23/18. Current financial responsibility documents are required to be submitted annually. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, and upload the required documents. This is a minor violation. 104 CCR 2711 UST operating permit application for facility and tank information not submitted or current. UST Facility Operating Permit Application BOE number is incorrect. The number listed is shown on the CDTFA website as closed and not valid. Immediately log into CERS, update the required information, and submit for review by the EHD. This is a Class II violation. 105 CCR 2632(d)(1), 2634(d) Failed to have an approved UST monitoring plan. The monitoring plan is not current and/or not approved by the EHD. -The monitoring plan for both tanks incorrectly list the leak detector model as "vaporless". The leak detectors on site are 99LD2000. -The monitoring plans for both tanks incorrectly list the monitoring panel for the tank, piping, and UDC monitoring as a TLS 350. The panel on site is a TLS 350 Plus. The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a minor violation. 201 CCR 2712(b) Failure to maintain monitoring, maintenance, and follow up action records. Maintenance and monitoring records for the last three years were not found on site. -Alarm logs are not being maintained for the sensor out alarms on site: L2 UDC 3/4 on 5/23/18 L4 UDC 7/8 on 5/6/19 L8 87/91 annular on 4/27/19 and 4/26/19 These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a minor violation.
