S A N J 0 A Q I I I Irl Environmental Health Department <br /> CCUNT <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Reach 02 1 6364 Lindbergh St, Stockton September 24, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 110 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br /> OBSERVATION: Designated facility signed and dated copies of Uniform Hazardous Waste Manifest(UHWM) <br /> 010076823FLE(10/13/2017)was not found on-site. <br /> REGULATION GUIDANCE: A hazardous waste generator shall keep a copy of each manifest signed in accordance <br /> with section 66262.23(a)for three years or until the generator receives a signed copy from the designated facility <br /> which received the waste. This signed copy shall be retained as a record for at least three years from the date the <br /> waste was accepted by the initial transporter. The hazardous waste generator must submit a copy of the manifest <br /> to the California Department of Toxic Substances Control (DTSC)within 60 days if the destination facility signed <br /> manifest copy is not received from the transporter or designated facility. <br /> CORRECTIVE ACTION: The violation was corrected during the inspection by the manifest being located on the <br /> DTSC website by the facility. No further action is required to correct this violation. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> The violations noted during the inspection were corrected on site. No follow up is required with the San Joaquin <br /> County Environmental Health Department. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. The report was emailed to the facility supervisor, Jeremy Galindo, at, <br /> <jeremy.galindo@calstar.org>. Starting September 1, 2018, all in-office CERS help will be provided at EHD <br /> hourly rate ($152).
