I have attached a simple 1-page document
that shows how to access these items at the store, which was part of the training provided to stores when we rolled out
this new tool. Based on the above, and the information provided in our Sep 28, 2018 letter, the Contingency Plan, along
with the Emergency Procedures Plan (EPP)Summary, and the HMBP submitted on CERS,which includes the CAL Office
of Emergency Services phone number, are the documents that support stores in the event of an emergency and are
available every time.
On regards to the records related to the training provided by the CVS Health environmental specialist Chris Yerzy, we
consider this training to be an additional training. Our team in California is dedicated to provide supplemental
training, reinforce compliance of the CVS hazardous waste program and assists stores with any regulatory concern .This
type of training is not included in the consolidated training records provided in our Sep 28,2018 letter since it is an
complementary of the annual required hazardous waste training. However, we recorded all this visits and trainings in
our Regulatory Database called Emerge, where our Specialists on the field enter the type of training and the topics
addressed during each visit. I am attaching the PFD document that was downloaded from the database, which shows
Mr.Yerzy visits and the training provided to the store.
Should you have additional questions, please let me know.
Thank you,
Adriana Morales I Senior Environmental Specialist, Corporate Environmental
c 401-644-8333 1 f 401-429-6507
CVS Health
CONFIDENTIALITY NOTICE:This communication and any attachments may contain confidential and/or privileged information for the use of the designated recipients named
above. If you are not the intended recipient, you are hereby notified that you have received this communication in error and that any review, disclosure, dissemination,
distribution or copying of it or its contents is prohibited. If you have received this communication in error, please notify the sender immediately by email or telephone and
destroy all copies of this communication and any attachments.
1
