On the Facility Employee Training Certificate dated 1/16/19, employee Judith Miranda was noted to assume responsibility as a facility employee on 11/6/18 but she was not initially trained until 1/16/19.All site employees must be trained prior to taking responsibility as a facility employee if after October 1,2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a minor violation. 204 CCR 2712(i)Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a minor violation. 205 CCR 2712(i), 2634(e)Failure to submit a current UST Response Plan and have it available onsite. An approved copy of the response plan was not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a minor violation. 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. A designated operator inspection was performed on 3/15/2019 and the next inspection was performed on 4/21/2019, 37 days later. The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. Ensure that all designated operator inspections are performed within the required time frames. The following was observed on the designated operator's monthly inspection reports: -Secondary containment testing was incorrectly noted, correct test date is 7/12/18. During the visual inspection,the designated operator shall check all required testing and maintenance for the UST system have been completed and shall check all required items on the report. Ensure that designed operators performing visual inspections at this facility are including all of the required information on the reports. This is a minor violation.
