No flappers were observed on site. <br /> -The tank forms list single-walled vent and vapor recover piping. Double walled fiberglass piping was observed in the fill <br /> sumps. It could not be determined if the piping was vent or vapor recovery. <br /> -The diesel tank form states there is no vapor recovery piping. The diesel tank was previously a gasoline tank. <br /> Confirm that the accurate information is submitted. <br /> Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, <br /> update the required information, and submit for review by the EHD. <br /> This is a Class II violation. <br /> 109 CCR 2637(a-d), 2712 Failed to test secondary containment, or failed to maintain test records for 36 months. <br /> UDCs 3/4 and 9/10 were installed on 7/20/17 and six month testing was due by January 2018. Testing was <br /> completed on 4/10/18, three months past due. Secondary containment testing is required six months after installation <br /> and at least every 36 months after. Ensure all testing is completed within the required time frames in the future. <br /> This is a Class II violation. <br /> 121 CCR 2712(f) Failed to submit a written response within 30 days of receiving an inspection report. <br /> An inspection was last done on January 22, 2018 and an inspection report was issued identifying information to be <br /> submitted to bring this site into compliance. This information was required to be submitted by February 21, 2018. <br /> The complete information was received on 3/21/18, one month past due. An operator that receives an inspection <br /> report shall have 30 days to submit a written response that includes a statement documenting corrective actions taken <br /> or proposing corrective actions which will be taken. Ensure that a written response documenting corrective actions <br /> taken or proposed is submitted within 30 days of receiving an inspection report. <br /> This is a Class II violation. <br /> 308 CCR 2636(f) Failed to maintain a continuous monitoring system that activates an audible and visual alarm. <br /> According to the technician, the diesel fill sump sensor passed initial testing, but then continued to go into and out of <br /> alarm for the remainder of the inspection. All monitoring equipment shall be maintained to activate an audible and <br /> visual alarm or stop the flow of product at the dispenser when it detects a leak. The sensor was replaced and retested <br /> during the inspection.
