No hazardous waste determination was made for this <br /> waste. <br /> -A blue container of unknown sandy waste was observed. No hazardous waste determination has been made. <br /> Any person who generates a waste shall determine if the waste is a hazardous waste. Immediately stop disposal of <br /> these wastes as non-hazardous, make a hazardous waste determination, and manage these wastes according the <br /> Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining how this waste <br /> was managed. <br /> This is a minor violation. <br /> 104 CCR 66262.34(d)(2) No modified contingency plan. <br /> An emergency coordinator and modified contingency plan information is lacking. There must be at least one <br /> emergency coordinator on site or on call to coordinate emergency response measures, and the following <br /> information must be posted by a phone: the name and phone number of the emergency coordinator; location of fire <br /> extinguishers, spill control equipment, and if present, fire alarm; and the phone number of the fire department, <br /> unless the facility has a direct alarm. Immediately appoint an emergency coordinator and post the required <br /> information by a phone. A form is provided that can be used for this purpose. <br /> This is a minor violation. <br /> 109 CCR 66262.23(x)(4) Failed to send generator manifest copies to DTSC within 30 days. <br /> Generator copies of manifest number 005947489SKS(3/29/17)was not sent to DTSC within 30 days of shipment of <br /> hazardous waste. The generator manifest copy shall be submitted to DTSC for every shipment on a manifest when <br /> California is either the generator state or the destination or consignment state. The generator manifest copy shall <br /> be mailed to: <br /> DTSC Generator Manifests <br /> P.O. Box 400 <br /> Sacramento, CA 95812-0400 <br /> Immediately mail a copy of the manifest listed and ensure that copies of all generator manifests are sent to DTSC <br /> within 30 days of shipping hazardous waste. <br /> This is a Class II violation. <br /> 110 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br /> Copies of uniform manifests signed by the TSDF for the disposal of hazardous waste for the past three years were <br /> not found on site. Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility <br /> and all receipts used in a consolidated manifesting procedure on site for three years and have them readily available <br /> for review.
