As of January 1, 2013, all businesses were required to submit all business plans electronically. <br /> Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to <br /> the EHD for approval. <br /> HMBP was submitted 9/24/2020. Corrected on site. <br /> This is a repeat violation, Class II. <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> Training records were not available at time of inspection. <br /> The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual <br /> refresher training.All employees shall be trained in safety procedures in the event of a release or threatened release <br /> of a hazardous material, including, but not limited to,familiarity with the following plans and procedures: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B)Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice,for the business site. <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br /> years. <br /> Immediately provide employee training for appropriate personnel and submit a copy of the training records to the <br /> EHD. <br /> This is a repeat violation, Class ll. <br /> Overall Inspection Comments: <br /> The inspection report has been amended by San Joaquin County EHD. This inspection report will replace the <br /> initial inspection report, provided to the facility on the day of the inspection. Violation#2 was accidentally marked <br /> during the inspection. Violation #2 has been removed from the inspection report. Violation#2 and #6 was <br /> accidentally marked as corrected on site, when violations were not corrected on site. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by October 24, 2020. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
