SAN <br /> J D �� I Environmental Health Department <br /> COUNTY <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> MAIN STREET SMOG 4403 E MAIN ST, STOCKTON October 11, 2018 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 105 CCR 66262.34(d)(2)No emergency coordinator. <br /> An emergency coordinator and modified contingency plan information is lacking. <br /> There must be at least one emergency coordinator on site or on call to coordinate emergency response measures, <br /> and the following information must be posted by a phone:the name and phone number of the emergency <br /> coordinator; location of fire extinguishers, spill control equipment, and if present,fire alarm; and the phone number <br /> of the fire department, unless the facility has a direct alarm. <br /> Immediately appoint an emergency coordinator and post the modified contingency plan by a phone. A form is <br /> provided that can be used for this purpose. <br /> This is a minor violation. <br /> 114 HSC 25160.2(b)(4)Consolidated manifest receipts failed to contain complete information. <br /> The consolidated manifest receipt for Crystal Blue Environmental on July 9, 2018 picked up 260 gallon of used oil; <br /> Used Cal EPA ID number of Hollywood Cars instead of the current business, Main Street Smog. <br /> All copies of each receipt shall contain all of the following information: <br /> -The name, address, identification number, contact person, and telephone number of the generator, and the <br /> signature of the generator or the generator's representative; <br /> -The date of the shipment; <br /> -The manifest number; <br /> -The volume or quantity of each waste stream received, its California and RCRA waste codes,the waste stream <br /> type listed in subdivision (c), and its proper shipping description, including the hazardous class and United <br /> Nations/North America(UN/NA)identification number, if applicable; <br /> -The name, address, and identification number of the authorized facility to which the hazardous waste will be <br /> transported; <br /> -The transporter's name, address, and identification number; <br /> -The driver's signature; <br /> -A statement, signed by the generator, certifying that the generator has established a program to reduce the <br /> volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator,to be <br /> economically practicable. <br /> Ensure that all future receipts for hazardous waste shipments using a consolidated manifesting procedure contain <br /> all required information, including the current ID number. <br /> This is a minor violation. <br /> 119 HSC 25404(e)(4) Failed to report program data in the California Environmental Reporting System (CERS). <br /> A submission to the California Electronic Reporting System (CERS)for the hazardous waste program has not been <br /> made. <br /> Beginning January 1, 2013, all businesses are required to submit all new(or any changes to existing)hazardous <br /> waste information online to the CERS at http://cers.calepa.ca.gov.
