The following is an itemized list of hazardous waste violations that have not been <br /> addressed for MAIN STREET GARAGE as of December 19 , 2018 . <br /> Open violations from November 07 , 2018 inspection <br /> Violation #102 - Failed to determine if a waste is a hazardous waste. <br /> A brake grinding machine was observed with the collection tray half full of brake shavings . The brake shavings are <br /> currently being recycled as scrap metal . Any person who generates a waste shall determine if the waste is a <br /> hazardous waste . Immediately make a hazardous waste determination for the brake shavings , and manage it <br /> according the Title 22 hazardous waste regulations . Submit a statement and supporting documentation explaining <br /> how this waste was managed . <br /> Violation #104 - No modified contingency plan . <br /> An emergency coordinator and modified contingency plan information is lacking . There must be at least one <br /> emergency coordinator on site or on call to coordinate emergency response measures , and the following <br /> information must be posted by a phone : the name and phone number of the emergency coordinator; location of fire <br /> extinguishers , spill control equipment, and if present, fire alarm ; and the phone number of the fire department, <br /> unless the facility has a direct alarm . Immediately appoint an emergency coordinator and post the required <br /> information by a phone . A form is provided that can be used for this purpose . <br /> Violation #119 - Failed to report program data in the California Environmental Reporting System ( CERS ) . <br /> A submission to the California Electronic Reporting System (CERS) for the hazardous waste program has not been <br /> made. Beginning January 1 , 2013 , all businesses are required to submit all new (or any changes to existing ) <br /> hazardous waste information online to the CERS at http : //cers. calepa . ca , gov . Be sure to include your hazardous <br /> waste activity in the Businesses Activities section in CERS in addition to any other relevant activities and required <br /> fields . <br /> Violation #403 - Failed to keep hazardous waste containers closed except when adding or removing <br /> hazardous waste . <br /> One black metal 55 gallon container of oily solids was observed behind the building without the lid secured onto the <br /> drum. All hazardous waste containers shall be closed at all times except when adding or removing waste . <br /> Immediately close these containers and ensure all hazardous waste containers are closed when not adding or <br /> removing waste . <br /> Violation #605 - Failed to completely label containers or portable tanks of hazardous waste. <br /> -One white plastic 55 gallon drum of used antifreeze was observed behind the building with a label that was too <br /> faded to be read . <br /> -One black metal 55 gallon drum of used oil was observed behind the building without the words " Hazardous <br /> Waste" , accumulation start date , hazardous properties , physical state and generator name and address on the <br /> label . <br /> All hazardous waste containers shall be marked with the following information : <br /> - the words " Hazardous Waste" <br /> - name and address of generator <br /> - hazardous properties <br /> - physical state <br /> - composition (contents) <br /> - accumulation start date <br /> Immediately label these containers and ensure that all hazardous waste containers are marked with all the required <br /> information . <br /> Violation #608 - Failed to properly manage used oil and fuel filters . <br /> One black metal drum of drained used oil filters was observed behind the shop with a lid that was not secured . <br /> Used oil filters shall be drained of free-flowing oil , stored in a closed , rainproof container, labeled as " drained used <br /> oil filters , " and marked with an accumulation start date . Immediately close the drum and ensure that all used oil <br /> filters are managed in this way . <br /> Page 1 of 1 <br />
