Kettleman Lane Lodi, CA 95240 Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> On site to conduct a routine inspection. <br /> 7. Contaminated used oil absorbent pads and oily rags are being disposed of in the trash without first <br /> determining hazardous properties. Any person who generates a waste shall determine if the waste is a <br /> hazardous waste. Immediately stop disposing of contaminated used oil absorbent pads and oily rags in <br /> the trash, make a hazardous waste determination for the contaminated used oil absorbent pads and oily <br /> rags, and manage it according to Title 22 hazardous waste regulations. Submit a statement and <br /> supporting documentation explaining how this waste was managed by 8-5-11. Minor-Notice to Comply. <br /> 11. There is oily debris on the vault floor between the west wall and the oil tank below bay #3. Facilities <br /> shall be maintained and operated to minimize the possibility of a fire, explosion, or release of hazardous <br /> waste to air, soil, or surface water which could threaten human health or the environment. Immediately <br /> clean up the oily debris and manage according to Title 22 hazardous waste regulations. Submit a <br /> statement and supporting documentation explaining how this waste was managed by 8-5-11. Class II <br /> Repeat Violation. <br /> 79. A broken universal waste(UW) lamp was stored in an open box at the northwest corner of the vault <br /> below bay #3. A UW handler shall manage lamps in a way that prevents releases of any UW or <br /> component of a UW to the environment. A UW handler shall contain any lamp in a container or package <br /> that is structurally sound, adequate to prevent breakage, and compatible with the contents of the lamp. <br /> Such a container or package shall remain closed and shall lack evidence of leakage or damage that <br /> could cause leakage under reasonably foreseeable conditions. Correct immediately by properly <br /> managing UW lamps in this way. Minor-Notice to Comply. <br /> 83. The UW Lamp was not labeled. A UW handler shall label or mark UW to identify the type of waste. <br /> Lamps (i.e., each lamp), or a container in which the lamp(s) are contained, shall be labeled or marked <br /> clearly with the following phrase: "Universal Waste-Lamp(s)". Correct immediately by properly managing <br /> UW lamps in this way. Minor-Notice to Comply. <br /> 84. The facility did not demonstrate when the broken UW lamp became a UW. A UW handler shall <br /> accumulate UW for no longer than one year from the date the UW was generated, or was received from <br /> another UW handler. A UW handler shall be able to demonstrate the length of time that the UW has <br /> been accumulated from the date it became a waste or was received. The UW handler may make this <br /> demonstration by: <br /> (1) Placing the UW in a container and marking or labeling the container with the earliest date that any <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: / Relived By: Title: <br /> EHD 23-02-003 Rev 08/10/10 CONTINUATION FORM <br />
