Manifest 011821828JJK was completed using the EPA ID and address of another facility. The EPA ID# for the business at this address is CAL000182645. The generator of any hazardous or extremely hazardous waste to be transported off-site or into California shall complete the generator and waste section and sign the manifest certification according to the instructions in the Appendix to this chapter. Ensure that the manifests are being properly completed including using the active EPA ID# assigned to the business at this address. 122, 123, & 124. Manifest numbers 009560128JJK, 009576320JJK, 009576324JJK, 009889518JJK, 009890669JJK, 009897344JJK, 010855959JJK, 011827090JJK, 012411521 JJK, 013075110JJK, and 011821828JJK were found without a signed off copy from the destination facility. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports shall be submitted to: ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
