If <br /> the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the <br /> facility to which the generator's waste was submitted within 60 days of the date the waste was accepted by the initial <br /> transporter, the generator shall submit a legible copy of the missing manifest, with some indication that the generator <br /> has not received confirmation of delivery. This information shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifests or prepare and submit the required information to DTSC. Submit <br /> to the EHD a copy of the signed off copy of the manifest or a copy of the information submitted to DTSC. <br /> This is a Class II violation. <br /> 111 CCR 66268.7(a) Failed to make determination for land disposal. <br /> At time of inspection there was no indication that a determination had been made if the hazardous wastes generated <br /> at this facility need to be treated prior to the disposal to land. The generator shall determine if the waste is restricted <br /> from land disposal prior to disposal. Ensure this is done for all future shipments. Provide this office with copies of all <br /> LDRs for the past 3 years. <br /> This is a Class II violation. <br /> 112 HSC 25160.2(b)(3) Failed to keep copies of consolidated manifesting receipts for three years. <br /> Copies of hazardous waste disposal records for the past 3 years were not found on site. Hazardous waste generators <br /> shall retain copies of all manifests signed off by the disposal facility and all receipts used in a consolidated <br /> manifesting procedure on site for three years and have them readily available for review. Immediately locate a copy of <br /> all manifests and receipts for the last three years, maintain them on site, and submit copies to the EHD. <br /> This is a Class II violation.
