Manifest numbers: 006985604FLE 006985629FLE 006085320FLE 008025056FLE 008025100FLE 001122842JJK 006085103FLE were found without a signed off copy from the destination facility. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 60 days of the date the waste was accepted by the initial transporter, the generator shall submit a legible copy of the missing manifest, with some indication that the generator has not received confirmation of delivery. This information shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifests or prepare and submit the required information to DTSC. Submit to the EHD a copy of the signed off copy of the manifest or a copy of the information submitted to DTSC. This is a Class II violation. 111 CCR 66268.7(a) Failed to make determination for land disposal. At time of inspection there was no indication that a determination had been made if the hazardous wastes generated at this facility need to be treated prior to the disposal to land. The generator shall determine if the waste is restricted from land disposal prior to disposal. Ensure this is done for all future shipments. Provide this office with copies of all LDRs for the past 3 years. This is a Class II violation. 112 HSC 25160.2(b)(3) Failed to keep copies of consolidated manifesting receipts for three years. Copies of hazardous waste disposal records for the past 3 years were not found on site. Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts used in a consolidated manifesting procedure on site for three years and have them readily available for review. Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit copies to the EHD. This is a Class II violation.
