The following is an itemized list of hazardous waste violations that have not been <br /> addressed for Becker Transmission and Auto Repair as of November06, 2018. <br /> Open violations from August 23, 2018 inspection <br /> Violation #101 - Managed a hazardous waste without an identification number. <br /> This facility's hazardous waste ID number is inactive. A hazardous waste generator shall not treat, store, dispose <br /> of, transport or offer for transportation, hazardous waste without a hazardous waste ID number. Go to the DTSC <br /> web site http://www.dtsc.ca.gov/IDManifest/PERMHWID.cfm for the form to reactivate the ID number. <br /> Immediately reactivate your state ID number and submit evidence to the EHD. <br /> Violation #110- Failed to keep signed copy of manifests from the designated facility for three years. <br /> Copies of uniform manifests for the disposal of hazardous waste for used oil (2015) and for used antifreeze (2018, <br /> 2017, 2016, and 2015)were not found on site. Hazardous waste generators shall retain copies of all manifests <br /> signed off by the disposal facility and all receipts used in a consolidated manifesting procedure on site for three <br /> years and have them readily available for review. Immediately locate a copy of all manifests and receipts for the last <br /> three years, maintain them on site, and submit copies to the EHD. <br /> Violation #113- Failed to keep copies of consolidated manifesting receipts for three years. <br /> Copies of hazardous waste disposal records for used oil (2015) and for used antifreeze (2018, 2017, 2016, and <br /> 2015)were not found on site. Hazardous waste generators shall retain copies of all manifests signed off by the <br /> disposal facility and all receipts used in a consolidated manifesting procedure on site for three years and have them <br /> readily available for review. Immediately locate a copy of all manifests and receipts for the last three years, maintain <br /> them on site, and submit copies to the EHD. <br /> Violation#117- Failed to submit a written response within 30 days of receiving an inspection report. <br /> An inspection was last done on 6/19/2015 and an inspection report was issued identifying information to be <br /> submitted to bring this site into compliance. This information was required to be submitted by 7/19/2015. This <br /> information has not been received resulting in a non-compliant status for this facility. An operator that receives an <br /> inspection report shall have 30 days to submit a written response that includes a statement documenting corrective <br /> actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting <br /> corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. Outstanding <br /> violations from the 6/19/2015 inspection include: <br /> - Managed a hazardous waste without a valid EPA ID number <br /> Violation#301 - Facility not maintained to minimize the release of a hazardous waste. <br /> A black 55-gallon drum labeled as and containing drained used oil filters was found with an open lid and used oil <br /> and oil filters sitting on top of the lid. A pile of stained, possibly oily, rags and dark staining was found beneath and <br /> adjacent to a piece of machinery on the south side of the shop. Facilities shall be maintained and operated to <br /> minimize the possibility of a fire, explosion, or release of hazardous waste to air, soil, or surface water which could <br /> threaten human health or the environment. Immediately clean the standing oil on the lid of the black used oil filter <br /> drum and clean up the stained rags and floor near the machinery on the south side of the shop and manage <br /> according to Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining <br /> how this waste was managed. <br /> Violation #302 -Failed to maintain required emergency equipment. <br /> The fire extinguisher located adjacent the used oil tank was found not fully charged. All facilities shall be equipped <br /> with properly charged fire extinguishers, an internal communication or alarm system, a device capable of calling <br /> outside emergency help, spill control equipment, and decontamination equipment at all times. Immediately remove <br /> the improperly charged fire extinguisher, replace it with a properly charged fire extinguisher, and maintain all <br /> emergency equipment on site. <br /> Page 1 of 2 <br />
