San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.sogov.org/ehd <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> GIANT DISCOUNT TIRE 330 E KETTLEMAN LN, LODI July 30, 2018 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 605 CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste. <br /> The following hazardous waste was found not properly labeled: <br /> -A 55-gallon drum of waste antifreeze, located outside behind the facility building,was labeled as hazardous waste, <br /> but was missing all of other required labeling information. <br /> -A 55-gallon drum of used absorbent and paper filters, located inside the facility,was missing the accumulation <br /> start date,the physical state, and the hazardous properties. <br /> -A 15-gallon drum of metal fines, located adjacent the two brake lathes inside the facility,was missing the <br /> accumulation start date. <br /> -A 30-gallon drum of used absorbent was missing all of the required hazardous waste labeling information. <br /> All hazardous waste containers shall be marked with the following information: <br /> -the words"Hazardous Waste" <br /> -name and address of generator <br /> -hazardous properties <br /> -physical state <br /> -composition (contents) <br /> -accumulation start date <br /> Immediately label these containers and ensure that all hazardous waste containers are marked with all the required <br /> information. <br /> This is a minor violation. <br /> 608 CCR 66266.130 Failed to properly manage used oil and fuel filters. <br /> Bills of lading for the recycling of used oil filters for the last three years were not found on site. A copy of each bill of <br /> lading must be kept on site for at least three years. Immediately locate a copy of all bills of lading for used oil filters <br /> for the last three years, maintain them on site, and submit copies to the EHD. <br /> Approximately 12 used metal oil filters were found sitting on top of the 55-gallon used oil filter drum. Used oil filters <br /> shall be drained of free-flowing oil, stored in a closed, rainproof container, labeled as"drained used oil filters,"and <br /> marked with an accumulation start date. Immediately drain and containerize the filters, label, and mark the date and <br /> ensure that all used oil filters are managed in this way. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> An inspection report was provided to the facility operator on the day of inspection. The report was amended by <br /> the EHD on 9/5/2018. The amended report replaces the initial report. <br /> Notes: <br /> -This facility has a new business owner as of 3/1/18. <br /> -The manager, Moe Salem, provided invoices for the used oil pick ups from American Valley Waste Oil, which <br /> amounted to 480 gallons since 3/1/18. He also provided an invoice for the pick up of used oil filters, which did <br /> not have a quantity in pounds or gallons.
