The following is an itemized list of hazardous waste violations that have not been <br /> addressed for SUPERLUBE PLUS INC as of October31, 2018. <br /> Open violations from August 27, 2018 inspection <br /> Violation#108 -Contingency plan is incomplete. <br /> The facility's contingency plan is incomplete. The contingency plan reviewed while on site was missing: <br /> -Description of actions facility personnel will take in response to fires or explosions at the facility <br /> -Description of arrangements made with local police departments, fire departments, hospitals, contractors and State <br /> and local emergency response teams to coordinate emergency services <br /> -List of names, addresses, and phone numbers (office/home/cell) of all persons qualified to act as emergency <br /> coordinator. Where more than one person is listed, one shall be named as the primary emergency coordinator and <br /> all others in order in which they will assume responsibilities; <br /> -List all emergency equipment, which includes location, physical description, and brief outline of its capabilities <br /> -Evacuation routes for facility personnel where there is a possibility of evacuation <br /> -Current telephone number for State Office of Emergency Services. <br /> The contingency plan must include: <br /> 1. Description of actions facility personnel will take in response to fires, explosions, or any sudden or <br /> non-sudden release of hazardous waste to air, soil or surface water at the facility; <br /> 2. Description of arrangements made with local police departments, fire departments, hospitals, contractors and <br /> State and local emergency response teams to coordinate emergency services; <br /> 3. List of names, addresses, and phone numbers (office/home/cell) of all persons qualified to act as emergency <br /> coordinator. Where more than one person is listed, one shall be named as the primary emergency coordinator and <br /> all others in order in which they will assume responsibilities; <br /> 4. List all emergency equipment, which includes location, physical description, and brief outline of its capabilities; <br /> 5. Evacuation routes for facility personnel where there is a possibility of evacuation; <br /> 6. Current telephone number for State Office of Emergency Services. <br /> If the facility has a Spill Prevention, Control and Countermeasures plan, or some other emergency or contingency <br /> plan, the owner or operator need only amend that plan to incorporate hazardous waste management provisions that <br /> are sufficient to comply with the requirements of this chapter. Immediately prepare or amend an existing plan to <br /> fulfill the requirements of this chapter. Hazardous waste generators shall retain copies of all <br /> manifests signed off by the disposal facility on site for three years and have them readily available for review. <br /> Immediately locate a copy of all manifests for the last three years, maintain them on site, and submit copies to the <br /> EHD. <br /> Violation #502 - Failed to comply with new hazardous waste tank system assessment requirements. <br /> Facility does not have a tank assessment for the 1000 gallon used oil tank. The owner or operator of a hazardous <br /> waste tank shall obtain a written assessment reviewed and certified by an independent, qualified, professional <br /> engineer, registered in California attesting that the system has sufficient structural integrity, is acceptable for the <br /> transferring, storing, and treating of hazardous waste, and that the tanks and containment system are suitably <br /> designed. Immediately have an assessment prepared for the 1000 gallon used oil tank. Submit a schedule <br /> showing when this will be accomplished and a copy of the final assessment to the EHD. <br /> Page 1 of 1 <br />
