The following is an itemized list of hazardous waste violations that have not been <br /> addressed for JIFFY LUBE #1478 as of August 13, 2018. <br /> Open violations from May 11, 2018 inspection <br /> Violation #114-Failed to keep signed copy of manifests from the designated facility for three years. <br /> -Copies of hazardous waste disposal records for 014058304JJK(6/5/15), 015145540JJK(10/13/15), 01451474JJK <br /> (12/17/15), 01454235JJK (3/7/16), 01454417JJK(5/2/16) were not found on site. <br /> - Manifests 014051072JJK(9/17/15), 014057947JJK (11/8/16), 017791896JJK(9/5/17)were found without <br /> destination facility signed off copies. <br /> -No disposal records for waste oil October 2017 through March 2018. <br /> Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts <br /> used in a consolidated manifesting procedure on site for three years and have them readily available for review. <br /> Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit <br /> copies to the EHD. <br /> Violation #115-Failed to comply with uniform hazardous waste manifest exception requirements. <br /> Manifests 014051072JJK(9/17/15), 014057947JJK(11/8/16), 017791896JJK(9/5/17) were found without a signed <br /> off copy from the destination facility. Hazardous waste generators shall retain copies of all manifests signed off by <br /> the destination facility on site for three years and have them readily available for review. If the generator did not <br /> receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the <br /> generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the <br /> generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status <br /> of the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date <br /> that the waste was accepted by the initial transporter, the generator shall submit an Exception Report that includes a <br /> legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the <br /> hazardous waste and the results of those efforts. Exception reports shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC. Submit to <br /> the EHD a copy of the signed off copy of the manifests or a copy of the exception report submitted to DTSC. <br /> Violation#120 -Consolidated manifest receipts failed to contain complete information. <br /> -The consolidated manifest receipt for 5/28/15 with manifest number 014058403JJK was missing the following <br /> required information: the waste code, the amount in pounds, waste minimization statement and destination facility. <br /> -The consolidated manifest receipt for 2/8/17 with manifest number 016766802JK was missing the following <br /> required information: the amount in pounds. <br /> All copies of each receipt shall contain all of the following information: <br /> -The name, address, identification number, contact person, and telephone number of the generator, and the <br /> signature of the generator or the generator's representative; <br /> -The date of the shipment; <br /> -The manifest number; <br /> -The volume or quantity of each waste stream received, its California and RCRA waste codes, the waste stream <br /> type listed in subdivision (c), and its proper shipping description, including the hazardous class and United <br /> Nations/North America(UN/NA) identification number, if applicable; <br /> -The name, address, and identification number of the authorized facility to which the hazardous waste will be <br /> transported; <br /> -The transporter's name, address, and identification number; <br /> -The driver's signature; <br /> -A statement, signed by the generator, certifying that the generator has established a program to reduce the <br /> volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator, to be <br /> economically practicable. <br /> Ensure that all future receipts for hazardous waste shipments using a consolidated manifesting procedure contain <br /> all required information. <br /> Page 1 of 1 <br />
