S A N J 0 A Q I I I Irl Environmental Health Department <br /> COUNT <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> ST JOSEPHS HOSPITAL 1800 N CALIFORNIA ST, STOCKTON October 26, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements. <br /> OBSERVATION: The designated operator failed to check and note all required items on the visual inspection <br /> reports.The incorrect and missing information included the following from the inspection reports: <br /> -11/08/2019, 01/06/2020, 02/04/2020, 03/05/2020, 04/07/2020, 05/07/2020: Section IX was not answered and <br /> overfill date was incorrectly documented as 10/082019 <br /> -12/08/2019&06/04/2020: Sections VII & IX were left blank and the spill bucket test date was incorrectly <br /> documented as 09/18/2019 <br /> -07/01/2020&07/29/2020:was not signed within 48hrs, Section IX was left blank and the overfill test date was <br /> incorrect <br /> -08/27/2020: overfill test date was incorrectly documented as 10/08/2018 <br /> -09/25/2020: Section IX was left blank <br /> -10/23/2020: contained inaccurate information. The report inaccurately listed dispensers and 87 and 91 tanks. This <br /> facility does not have dispensers and only stores diesel. <br /> REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by November 25, 2020. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
