The following is an itemized list of hazardous waste violations that have not been <br /> addressed for DOLLAR TREE #01260 as of October 09, 2020. <br /> Open violations from May 06, 2019 inspection <br /> -A bag labeled"Hazardous Waste"containing an empty 9.5 fl. oz. container of Pine-Sol Multi-Surface Cleaner and <br /> a roll of paper towels,which appeared to be contaminated with an unknown liquid,was found in an open Dollar Tree <br /> basket.The basket containing the Pine-Sol was found on the ground near shelves of baskets containing items to be <br /> placed back on the sales floor, as well as damaged items. Lacie, did not know how long the HW bag with the <br /> Pine-Sol had been in the basket. The scanner indicated the Pine-Sol should be placed in a Corrosive Acids(CA) <br /> HW tote. Lacie took the HW bag with the Pine-Sol and contaminated paper towels to the HW storage area, but did <br /> not placed these item in a CA HW tote at the time of inspection. (IMG 5905-5909, 5921-2) <br /> -A bag labeled"Hazardous Waste"containing an open Blue Ribbon Chocolate Sundae Cone was found in a basket <br /> located on a shelf of baskets containing items to be placed back on the sales,floor, as well as damaged items. <br /> According to Lacie, she did not know who placed the ice cream into the HW bag.The HW bag with the ice cream <br /> was not placed in a HW tote at the time of inspection. (IMG 5910) <br /> -A bag labeled"Hazardous Waste"and"OX,"containing an empty damaged 96 fl. oz. container of The Home Store <br /> Bleach and used paper towels,was found on the top shelf of the HW storage area. Lacie did not know who had <br /> placed the empty container on the shelf, nor what happened to the missing liquid that was once in the container. <br /> Used absorbent was not found to account for the missing liquid. The scanner indicated the bleach container should <br /> be placed in an Oxidizers(OX) HW tote. Laci placed the bagged item into an OX HW tote. (IMG 5911-5) <br /> -A Corrosive Bases(CB)HW tote was found with an accumulation start date of 4/17/2012(past the 180-/270-day <br /> storage limit)and the contents were not marked off on the tote's HW label. The CB HW tote contained the following <br /> (IMG 5915-20): <br /> o A HW bag containing several empty packets of GS Neutral Cleaner Concentrate pH Neutral Floor Cleaner. <br /> According to Lacie, the floor cleaner is used to clean the facility floor, and the empty packets are placed in a CB HW <br /> tote based on the information in the guide titled "Products To Be Treated As Hazardous Waste,"which was posted <br /> by the HW storage area. <br /> o An empty 20 fl. oz. container of LA's Totally Awesome All-Purpose Concentrated Cleaner. Lacie did not know <br /> what happened to the missing liquid once contained in the container. Used absorbent was not found to account for <br /> the missing liquid. <br /> Somphon Panyasy, store manager, marked the contents on the CB HW label, but the accumulation start date of <br /> 4/17/2012 remained (IMG 5923). <br /> -An Aerosols, Flammables, and Combustibles(AF) HW tote was found with the physical state and the contents not <br /> marked on the tote's HW label. The AF HW tote contained an aerosol can in a HW bag (IMG 5924-5). Somphon <br /> marked off the contents on the HW label, but did not mark the physical state(IMG 5926). <br /> -A STOU HW tote was found with the contents not marked on the tote's HW label.The STOU HW tote contained <br /> the following (IMG 5927, 5931-8): <br /> o A HW bag labeled"CB"and containing a partially empty 32 fl. oz. container of LA's Totally Awesome Oxygen <br /> Orange All-Purpose Degreaser.The scanner indicated the degreaser should be placed in a CB HW tote. Somphon <br /> did not know what happened to the missing liquid that was once in the container. Used absorbent was not found to <br /> account for the missing liquid. Somphon placed the degreaser into a CB HW tote. <br /> o A HW bag containing a box of Color Eazy Permanent Cream Hair Color,which contained a tube of permanent <br /> hair color and a bottle of developer.When the barcode on the box was scanned,the scanner indicated the items in <br /> the box should be placed in an STOU HW tote. However, the ingredient list on the box stated that the cream hair <br /> color contains ammonium hydroxide and the developer contains hydrogen peroxide.According to the Dollar Tree <br /> California Training Guide on Hazardous Waste Handling and Processing Procedures items containing ammonium <br /> hydroxide are classified as CB and items hydrogen peroxide are classified as OX. The hair color and developer <br /> were not placed in separate HW totes at the time of inspection. <br /> -A STOU HW tote was found containing several items, however the contents was not marked on the tote's HW <br /> label. One of the items in the tote was a HW bag labeled"STOU"containing a Hybrid Pocket Lighter.The scanner <br /> indicated the lighter should be placed in an AF HW tote. Somphon placed the lighter into the AF HW tote. (IMG <br /> Page 3 of 7 <br />
